Viêm da tiết bã là một bệnh lý cần dùng thuốc, chứ không phải là da nhờn do tuyến bã tiết nhiều chỉ khắc phục tạm thời bằng mỹ phẩm.

Nguyên nhân

Chưa hoàn toàn sáng tỏ song có nhiều giả thiết có tính thuyết phục:

Do thiếu, rối loạn hormone: bằng chứng: viêm da tiết bã và vẩy phấn da dầu thường gặp ở lứa tuổi rất nhỏ (10% ở trẻ trai, 95% ở trẻ gái). Trẻ dưới 3 tháng mắc nhiều, giảm nhanh khi 1 tuổi, giảm chậm hơn trong 4 tuổi tiếp theo, rồi hết hẳn. Bệnh lại tái hiện rầm rộ ở tuổi trưởng thành (có tỷ lệ cao hơn ở người lớn tuổi). Ứng với thời kỳ mắc nhiều là thời kỳ thiếu hay rối loạn hormone.

Do nấm: bằng chứng: ở nơi có chủng nấm Malsssezia tăng sinh mạnh, thì ở đó có viêm da tiết bã (theo Mastrolonardo M - 2003, Schwartz RA- 2004). Loài nấm này thường chiếm ưu thế ở những vùng có nhiều tuyến bã (đầu thân, lưng trên). Dùng kháng nấm chống lại sự tăng sinh của vi nấm, bệnh được cải thiện (Heng MC -1990).

Do suy giảm miễn dịch: bằng chứng: một số bệnh làm suy giảm miễn dịch ở trẻ nhỏ hay suy giảm miễn dịch do AIDS làm bệnh dễ xuất hiện (DunicI - 2004).

Do nguồn gốc thần kinh: bằng chứng: bệnh thường biểu hiện dưới dạng vẩy da đầu (gàu) hoặc hồng ban ở vùng nếp gấp mũi, môi trong giai đoạn bị stress, mất ngủ. Tìm thấy một số mối liên hệ giữa viêm da tiết bã với bệnh Parkinson, động kinh, chấn thương hệ thần kinh trung ương, liệt dây thần kinh mặt, bệnh rỗng tủy sống do các thuốc liệt thần kinh gây ra kèm theo chứng ngoại tháp, các rối loạn thần kinh khác, bao gồm sau tai biến mạch máu não. Tuy nhiên, chưa tìm thấy chất dẫn truyền thần kinh gây bệnh.

Biểu hiện

Viêm da tiết bã có một số dấu hiệu:

Ở thanh thiếu niên và người lớn: bắt đầu bằng những vảy mỡ nhờn ở da dầu kèm nổi hồng ban và đóng vảy ở vùng rãnh môi - mũi hoặc vùng da sau tai. Cũng có ở vùng khác như: vùng lông mày, vùng râu mọc, vùng dưới vú, chỗ nếp gấp vùng thân, trên ngực, có khi cả ở giữa mặt. Có khi dạng hình cánh hoa, kèm theo có sần nang lông và quanh nang lông màu đỏ nâu, có khi dạng hình vẩy phấn, thành từng dát và mảng khắp người như vảy phấn hồng.

Ở trẻ nhỏ: vùng đỉnh đầu sẽ có đóng vảy nhờn như mỡ nhưng không gây ngứa, không gây nứt, rịn nước (như viêm da cấp), vẩy có màu trắng, trắng xám hay vàng, hay xuất hiện vào tuần thứ 3 - 4 sau sinh.

Tuy dễ nhận thấy, nhưng những dấu hiệu này cũng làm nhầm sang một số bệnh khác (như vẩy nến, chàm thể tạng, nấm tóc, nấm candida, mụn trứng cá).

Một số điểm chung về thuốc

Thuốc điều trị viêm da tiết bã hướng vào việc chống lại các nguyên nhân trên (xem chi tiết các loại thuốc ở dưới).

Thuốc thường dùng ngoài, dưới dạng dầu gội đầu hay dạng bôi trên da, ở nồng đô thấp, ít lần trong tuần, nên ít gây hại. Cần chọn lựa kỹ lưỡng, tránh dùng nhầm qua dạng có nồng độ cao hơn (chỉ dành dùng cho bệnh khác). Với riêng corticoid cần thận trọng, chỉ dùng trên vùng mặt viêm tiết bã khi có chỉ định của thầy thuốc da liễu.

Thuốc kháng viêm

Viêm da tiết bã vùng da đầu ở người lớn thường khởi đầu điều trị bằng một corticoid hoặc một chất ức chế canxi - thần kinh (cakcineurin) tại chỗ. Chúng thường dùng dưới dạng dầu gội đầu, cream, lotion.

- Dầu gội chứa corticoid: thường dùng loại dầu gội chứa fluocinolon.

- Cream bôi tại chỗ: thường dùng loại cream chứa các chất fluocinolon, betamethason, desonid. Hay dùng nhất là desonid, đó là một corticoid không có fluor (hydroxyprednisolon acetonid) có hoạt tính chống viêm, chống ngứa và ức chế quá trình sinh tổng hợp ở biểu bì và bì rất mạnh. Dùng trong viêm da tiết bã (và một vài bệnh khác như: eczema do tiếp xúc, viêm bì không điển hình liken hóa). Trong viêm da tiết bã thường dùng ở nồng độ thấp (0,05%).

- Chất ức chế calcineurin: thường dùng mỡ tacrolimus, cream pimecrolimus. Các chất này có tính kháng viêm, diệt nấm và không có nguy cơ làm giảm sức đề kháng của da và teo da như các corticoid. Có khi chúng cũng được dùng kết hợp với corticoid bôi tại chỗ. Hai thuốc này được dùng hàng ngày trong viêm tiết bã ở vùng mặt, vùng tai. Chỉ dùng dưới dạng thuốc bôi. Cần phân biệt với tacrolimus dạng thuốc uống hay truyền tĩnh mạch (dùng với mục đích chống thải loại trong ghép thận).

Thuốc chống nấm

Thường dùng chất chống nấm phổ rộng ketoconazol (dạng gel), mỗi ngày một lần, kết hợp với corticoid (desonid) mỗi ngày một lần, kéo dài trong hai tuần.

- Dầu gội chứa ketoconazol dùng cho viêm tiết bã da đầu, mỗi tuần dùng 3 lần cream (dạng gel) ketoconazol có thể dùng kết hợp với desonid tại chỗ trong viêm tiết bã da mặt, mỗi ngày một lần, dùng trong 2 tuần.

- Dầu gội selenium sulfid: có tính kháng nấm phổ hẹp. Dùng cho viêm tiết bã da đầu (và bệnh khác như lang ben) với dịch treo có nồng độ thấp 2 - 3%. Cách dùng: dùng nước nóng thấm ướt tóc. Lắc kỹ dịch treo và đổ vào lòng bàn tay 1 - 2 thìa cà phê dịch treo xát nhẹ vào da dầu (không làm dây vào mắt). Để 2 - 3 phút rồi gội đầu bằng nước sạch. Làm lại như trên một lần nữa. Dùng 2 - 3 lần như thế mỗi tuần.

Ngoài các chất trên, có thể dùng các thuốc kháng nấm tại chỗ fluconazol hay ciclopirox hoặc dùng thuốc kháng nấm uống terbinafin.

Chất tiêu sừng

Chất tiêu sừng (keratolytic) là các chất được dùng trong các phác đồ điều trị viêm da tiết bã cổ điển. Đó là những chất có tính tiêu sừng, nhưng không có tính kháng nấm như: acid salicylic, pyrithion. Riêng pyrithion - zinc vừa có tính tiêu sừng, vừa có tính kháng nấm (nhưng không kháng nấm đặc hiệu).

- Acid salicylic: làm tiêu lớp sừng trên da, dùng dưới dạng dầu gội đầu cho viêm da đầu tiết bã mỗi tuần 2 – 3 lần, hay bôi lên viêm da tiết bã ở vùng da khác mỗi ngày 2 - 3 lần.

- Pyrithion - zinc: là dẫn chất mercapto - pyridin. Vừa có tính tiêu sừng vừa có tính kháng nấm. Trong viêm da đầu tiết bã nhờn, dùng dạng dầu gội 1,7%, nên để dầu gội trên tóc ít nhất là 5 phút cho dầu gội thấm vào da đầu, mỗi tuần có thể gội 2 lần. Trong viêm da tiết bã nhờn, dùng dạng nhũ tương 0,5%, mỗi tuần 2 - 3 lần. Loại này có thể dùng cho vùng da mặt nhưng không được làm dây vào mắt.

Viêm da tiết bã có tần số mắc cao, đặc biệt ở trẻ nhỏ, tuổi trưởng thành, người có các bệnh thần kinh liên quan. Tuy nhiên, người bệnh ít khám, có khi nhầm là do tuyến bã tăng hoạt động làm cho da nhờn, tự khắc phục tạm thời bằng mỹ phẩm. Đúng ra, bệnh cần được khám tại chuyên khoa da liễu, phải điều trị bằng dược phẩm.

Protopic acrolimus topical (ta KRA lih muss TOP ik al)


What is Protopic?

Protopic is a topical macrolide immunosuppressant. It works by decreasing cell activity in the body's immune system which helps to slow down the growth of atopic dermatitis (eczema) on your skin.
Protopic is used to treat severe atopic dermatitis when other medications have not worked well.
Protopic may also be used for purposes other than those listed here.

Important information about Protopic

Do not use Protopic on a child 2 years of age or younger.
A small number of people have developed skin cancer after using Protopic or pimecrolimus (Elidel) skin medications. However, it is not known if either of these medicines causes skin cancer. Talk to your doctor about your individual risk.
Use this medication only on the skin, exactly as it was prescribed for you. Do not use it in larger doses or for longer than recommended by your doctor. Protopic is not for long-term use.
If you are using this medicine on a child younger than 16 years old, use only the 0.03% ointment. The 0.1% ointment is for adults and children who are at least 16 years old, but is too strong to use on younger children.
Before using this medication, tell your doctor if you are pregnant or breast-feeding.
Avoid sunlight, sun lamps, tanning beds, and phototherapy treatments with UVA or UVB light. If you must be outdoors, wear loose clothing over the skin areas treated with Protopic. Do not use sunscreen on treated skin unless your doctor has told you to.
Talk to your doctor if your skin condition does not improve after using Protopic for 6 weeks.
If you miss a dose, use the medication as soon as you remember. If it is almost time for the next dose, skip the missed dose and wait until your next regularly scheduled dose. Do not use extra medicine to make up the missed dose.
There may be other drugs or skin products that can affect Protopic. Tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor.
Stop using this medication and get emergency medical help if you think you have used too much medicine or if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
Less serious side effects are more likely, and you may have none at all. Talk to your doctor about any side effect that seems unusual or is especially bothersome.

Before using Protopic

Before using Protopic, tell your doctor if you have:
  • skin cancer or a skin infection (including herpes or chicken pox);
  • Netherton's syndrome (a genetic skin disorder);
  • a weak immune system (from cancer treatments, HIV/AIDS or certain medicines such as steroids);
  • kidney disease; or
  • swelling, redness, or irritation of large areas of your skin.
If you have any of these conditions, you may not be able to use Protopic or you may need a dosage adjustment or special tests during treatment.
Protopic can lower the blood cells that help your body fight infections. This can make it easier for you to get sick from a virus such as chicken pox or herpes (cold sores or shingles). Tell your doctor if you have been exposed to any illness.
FDA pregnancy category C: This medication may be harmful to an unborn baby. Do not use Protopic without telling your doctor if you are pregnant. Tell your doctor if you become pregnant during treatment. Protopic passes into breast milk and may harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I use Protopic?

Use Protopic only on the skin, exactly as it was prescribed for you. Do not use it in larger doses or for longer than recommended by your doctor. Protopic is not for long-term use.

Wash your hands before and after using Protopic, unless you are using the medication to treat a hand condition.
Apply the medicine only to clean, dry skin. Use the smallest amount needed to apply a thin layer to the skin areas diagnosed with atopic dermatitis. Rub in gently. Do not cover the treated skin with a bandage.
Do not bathe, shower, or swim right after applying Protopic. Water may wash off the medicine.
Right after applying Protopic, you may need to rub in a moisturizing cream or lotion to keep your skin from getting too dry. Ask your doctor about which cream or lotion to use.
Talk to your doctor if your skin condition does not improve after using Protopic for 6 weeks.
Store Protopic at room temperature. Do not freeze. Keep the cap on the ointment tube when not in use.

What happens if I miss a dose?

If you miss a dose, use the medication as soon as you remember. If it is almost time for the next dose, skip the missed dose and wait until your next regularly scheduled dose. Do not use extra medicine to make up the missed dose.

What happens if I overdose?

Seek emergency medical attention if you think you have used too much of this medicine. Protopic is not expected to cause overdose symptoms.

What should I avoid while using Protopic?

Avoid using any other skin product that has not been recommended by your doctor.
Avoid sunlight, sun lamps, tanning beds, and phototherapy treatments with UVA or UVB light. If you must be outdoors, wear loose clothing over the skin areas treated with Protopic. Do not use sunscreen on treated skin unless your doctor has told you to. Avoid drinking alcohol. Drinking alcohol while you are using Protopic may cause your skin or face to feel hot and become flushed or red.

Protopic side effects

Stop using Protopic and get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
Stop using Protopic and call your doctor if you have:
  • worsened skin symptoms;
  • signs of a skin infection (redness, swelling, itching, oozing);
  • swollen glands; or
  • fever, chills, body aches, flu symptoms.
Less serious Protopic side effects are more likely to occur. Continue using Protopic and talk with your doctor if you have any of these side effects:
  • burning, stinging, tingling, or soreness of treated skin (especially during the first few days of treatment);
  • swollen hair follicles;
  • acne;
  • upset stomach;
  • muscle pain;
  • headache; or
  • feeling more sensitive to hot or cold temperatures.
Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome. You may report side effects to FDA at 1-800-FDA-1088.

What other drugs will affect Protopic?

Before using Protopic, tell your doctor if you are using any of the following drugs:
  • cimetidine (Tagamet).
  • erythromycin (Ery-Tab, E-Mycin, E.E.S. and others);
  • an antifungal medicine such as ketoconazole (Nizoral), itraconazole (Sporanox), or fluconazole (Diflucan); or
  • a heart medicine such as verapamil (Calan, Verelan), diltiazem (Cardizem, Dilacor XR, Tiazac), nifedipine (Adalat, Procardia), or nicardipine (Cardene).
There may be other drugs or skin products that can affect Protopic. Tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor.
